Overview of services
The Antimicrobial Stewardship and Evaluation Team (ASET) are dedicated to enhancing the quality of antimicrobial use throughout Duke University Hospital. ASET works collaboratively with clinical pharmacists, medical staff and other healthcare workers to provide safe, efficacious, and cost-effective drug therapy to optimize outcomes in patients with infectious diseases.
The Duke University Hospital Antimicrobial Stewardship and Evaluation Team (ASET) is co-chaired by Rebekah Moehring, MD, and Rebekah Wrenn, PharmD, BCIDP. ASET is a part of the Duke Center for Antimicrobial Stewardship and Infection Prevention (DCASIP), the Duke University Health System Pharmacy Medication Management Committee (PMMC), and the Duke University Hospital Pharmacy and Therapeutics (P&T) committee. Our team of pharmacists and physicians have worked together to establish ASET as a highly effective and efficient team since 2013. Put simply, patient care is safer at DUH because of ASET activities.
Major Stewardship Initiatives
ASET team members direct all antimicrobial stewardship activities at DUH, including active interventions such as post-prescription review, restriction, and penicillin skin testing. The direction of ASET is driven by a leadership group of adult and pediatric infectious diseases physicians and pharmacists. The leadership group oversees multidisciplinary monthly ASET committee meetings
- Prospective audit and feedback using the Epic Antimicrobial Stewardship Navigator
- Pharmacist-driven penicillin allergy assessments and penicillin skin testing
- Rapid diagnostic testing interpretation in collaboration with microbiology
- Pharmacokinetic and pharmacodynamic dose optimization
- Intravenous to oral switch program
- Duration of therapy guidance
- Antibiotic prophylaxis in surgery guidelines for adults and pediatrics
- Development of infectious diseases guidelines
- Analysis of prescribing and utilization patterns using the Antimicrobial Stewardship Assessment Portal (ASAP)
- Medication and antimicrobial-related clinical pathways and guideline reviews in collaboration with the Center for Medication Policy
- Web-based Clinical Decision Support (customid.org)
- Didactic and experiential training of present and future physicians and pharmacists
