Pharmacy Residency Application Information

Thank you for your interest in Duke’s Pharmacy Residency Program.  All of the Duke Pharmacy Residency Programs except for the Drug Information residency utilizes the online PhORCAS Application System.

Please note: Residency applicants must be authorized for employment in the United States at the time they apply for the residency. Duke University Health System Pharmacy Residency Program does not sponsor applicants for work visas at this time.

Application Deadlines

Health System Pharmacy Administration residency applicants also must complete a UNC graduate school application in addition to the residency program application.

Complete applications include the following:

  • An application form submitted through the PhORCAS Application Portal
  • Cover letter (approximately 1 page)
  • Curriculum Vitae
  • Official pharmacy college transcripts
  • PhORCAS Standardized Recommendations
    • PGY1 programs - Four (4) letters of recommendation (at least two (2) from clinical preceptors)
      • Preceptors should comment on the following:
        • (1) Describe practice site (ex: number of beds, trauma level, pharmacy practice model, etc)
        • (2) Rotation description (number of patients followed, rounding vs non-rounding)
        • (3) Comment on clinical skills (if applicable to rotation)
    • PGY2 programs:

 

PGY2 Program 

Application Requirements

PGY2 Cardiology

  • Three (3) letters of recommendation (at least one (1) reference must be from a clinical preceptor). Reference writers should complete the standardized PhORCAS reference form only.

    • Clinical preceptor(s) should comment on the following:

      1) Describe practice site (ex: number of beds, trauma level, academic vs. community setting, pharmacy practice model, etc.)

      2) Rotation description (number of patients followed, resident responsibilities, team structure, rounding vs. non-rounding)

      3) Comment on clinical skills

  • Supplemental Essay (required):

    • In a separate document, please answer the following question (Please limit the total response to 750 words):

      • Please provide a detailed overview of your staffing requirements during your PGY1 year, including the total number of hours spent on staffing per month. Be sure to include weekend staffing locations and frequency, hours spent on weekend staffing, evening staffing requirements, and any on-call components (if applicable). Additionally, outline your responsibilities while fulfilling these staffing requirements, such as order verification, clinical consultation, documentation requirements, emergency response coverage, patient education, and any other relevant tasks.

 
 
 

 

  • National Matching Services/ASHP Residency Matching Program Number

All applicants must be registered with the National Matching Service.

Applicants selected for an interview will be contacted by the residency program director to discuss available dates.  PGY1 Acute Care virtual interviews will be held between January and March.


Drug Information Residency

The Drug Information Residency does not utilize the PhORCAS Application Process.  The application should include a letter of intent, curriculum vitae, and three letters of recommendation. Application materials must be submitted electronically using this link no later than October 7th, 2025. Letters of recommendation should be submitted no later than November 1st 2025.
